Care Solutions for Elders is a RN geriatric care management team providing elder care advocacy, guidance, case management, and training services for care givers. We are known for providing the best Home Care Coordination for your elder and family. Our coordination will ensure a safe home care program for elders to continue to live in their homes and in their communities.  Our professional RN team assists elders, families, and caregivers receive the information and resources they need to make safe decisions along the life span.

Elder Care Advocacy Services

As certified members of the National Association of Professional Geriatric Care Managers, we understand the aging adult. As registered nurses we function as  Health Care Advocates. We understand the health care system making sure your health care and health care benefits are maximized with maximized outcomes. Our RN's are familiar with the hospitals, assisted living, skilled nursing and board and care homes; including policies and procedures, medical treatment, insurances, and how to navigate the health care system. Our expertise in elder care ensures compassionate, respectful, confidential and reliable care.

Elder Care Guidance

Our consultation provides you with information and resources when caring for an elder. A phone or face-to-face consultation can provide you with objectivity, resources, and information.  The consultation can review community resources, insurance, health care facilities, and health care programs. We also understand the health care system and care costs to ensure your care plan is parallel to your financial plan.

Elder Care Training and Education

Our case management and training labs are in the same building allowing us to provide more collaboration, cohesiveness and continuity in caring for the elderly. We provide training to families and caregivers at our local facility focusing on:

• certified personal care attendant program
• memory care specialist program
• basic skills program

Each elder and his or her family have a unique place in our hearts.  We understand they want to continue to feel useful, independent, and, above all, in control, even when their health is compromised. Our clients include families, friends, and relatives and others facing elder care challenges.
