Cynthia Seymour

Cynthia is a clinically trained health navigator and provides guidance and solutions for the complexity of aging.

She is teaming up with Tina to blend the best in RN case management with prevention and wellness. They are guides and managers offering proven resources and support families throughout the care cycle: hospital, facility, rehab and home.

Prior to starting Care Navigate, an elder care consultancy,Cynthia helped transform patient care for Sonoma Community Clinics through case management with self-care coaching for chronic disease: diabetes, hypertension and cardio-pulmonary conditions. Care Navigate at http://carenavigate.com/

Cynthia has a passion for movement as medicine, the cornerstone for improving health and well-being as well as memory. She continues to volunteer with Senior Access, a memory club Marin families have relied on for decades. Trained through Alzheimer’s Foundation of America, Cynthia conducts memory assessments to get a reference point for further detection and help. She also delivers workshops on brain health and wellness at senior centers throughout the Marin community.

Cynthia has a passion for movement as medicine, the cornerstone for improving health and well-being as well as memory.

Cynthia is an accredited Health Navigator from Sonoma State University with bachelors and masters degrees from UC Davis and UC San Diego. She is a certified nurse assistant and senior fitness leader. She has also become a palliative and hospice benefits specialist, focused on the importance of early advanced care planning and conversations about end of life wishes and values.
