We assist in making sure our clients are properly prepared for the unexpected as in surgeries and the expected as in procedures or treatments. We also help our clients with compliance to remain stable, in their discharge instructions and the follow up care to avoid the preventable admissions which is always so stressful the elder and for the family.

Finally we act as a single resource for patients to call when they have questions or concerns. We coordinate transitions between the MD office and the hospital including rehab and with home care providers. Our job is to educate and coordinate for the best outcomes.

The CARES MODEL was designed to help our clients understand the aging process. Most of our clients see us first to understand the complex and maze of health care. We then review in depth the care needs and provide a tho¬≠rough assessment identifying key components for safety and wellness in the living environment. Our medical knowledge and training can assess the individual's specific needs and help you understand your medical care plan so you can reach your goals. Our education and advocacy skills ensures that your care is communicated  and coordinated with your goals among all the team players. We develop and provide a comprehensive guided care plan for families focusing on the elder to remain in the least restrictive environment while balancing their ADL (Activities of Daily Living) with medical requirements. Our focus is to educate the family to get the results they are looking for. 

Our recommendations may include suggestions for home modification, purchase of home safety equipment, medical referrals, information regarding the cost of long-term care options, caregivers, local support, etc. While exploring the various care options we will work to ensure that the care plan runs parallel to your financial plan. Once in our family we can continue to evaluate and adjust as necessary the needs of your elder.  We work to create a plan that is clear, consisting of best safe practices that ensure the welfare and happiness of your elder and provide you peace of mind.  We believe you should have choices.
